The supernumerary teeth are defined as the teeth that are excess of the normal
number. Especially, mesiodens refers to a supernumernary tooth that is present in the
midline of the anterior maxilla. Such supernumerary teeth can casue midline diastema.
The diastema is defined as a space greater than 0.5 millimeter between the proximal
surfaces of adjacent teeth. Diastemas usually are a normal stage in the development of
the dentition, specially between the ages of 7 and 12 years. Usually these diastemas
close completely after eruption of permanent maxillary canines. But sometimes diastemas
remain unclosed. Therefore differential diagnosis in necessary between physiologic
diastema and pathologic one. The patient, 8-year-old boy, with the chief complain of
midline diastema and excessive teeth rotation visited the department of pediatric
dentistry of seoul national university hospital. The supernumerary teeth were removed
surgically. The midline diastema and teeth rotation were treated by the fixed orthodontic
applinace. And then frenectomy, fibrotomy and gingivoplasty were performed. The
periodic recall check and comprehensive orthodontic treatment are recommended.
